• Provider-specific patient communications — appointment reminder letters, test results, depart summaries, release of health information, billing statements, etc.
• Select signage for building exteriors and interior spaces to clearly identify who’s providing health care services at the location.
• Accreditations, designations and other recognitions based on the delivery of care at our medical centers or hospitals (Magnet, Level I trauma center, U.S. News, etc.)

Level 3 major entity logo files have been developed with all our provider names “locked up” with our master brand. Failure to include the correct logo or name may put us at risk of treating these locations as “provider-based,” as defined by the Centers for Medicare & Medicaid Services (CMS), and reduce the payments we receive.
